Gynecology / Women's Health - Information for your Visit

SIMED Women's Health strives to provide each of our patients with the attention they deserve. While we have allotted times within our schedules for same day appointments, we ask that you reserve these times for serious health issues. This will allow us to spend time with our very sick patients and schedule an appropriate amount of time to address your concerns. Please remember that we schedule appointments based on your current health concern so be sure to tell our Clinic Assistants as much information about your problem as you feel comfortable. This will assist them in scheduling the appropriate amount of time for your appointment.

If you should have to cancel your appointment, we do request 24 hours notice. This will allow us to contact other patients who may wish to be seen sooner and not let an appointment slot go to waste.

We try diligently to stay on time but because each patient has different needs or emergencies may arise, we may run slightly behind. You may always feel free to call our office ahead of time to find out if your physician is on schedule.

For your first visit:

If this is your first visit with SIMED Women's Health, welcome! We look forward to assisting you with your healthcare needs.

In order to expedite your visit you may download your new patient paperwork and fill it out at home. By filling it out at home you have the opportunity to save time, but more importantly you can fill out your health information in a quiet environment where you will have access to your health history, medications, reason for your visit and other important information. The more information you can provide your physician, the easier it will be for your physician to assess your current state of health.

Be sure to bring with you

  • a current photo identification
  • your current insurance card
  • Your New Patient Paperwork. (If you haven’t filled out your new patient paperwork, you will be asked to fill it out when you arrive)
  • Cash, Check, Credit or Debit card to pay any patient due portion of your bill.

You may also want bring

  • a list of questions for your physician
  • a history of your current symptoms
  • any recent medical records and test results from other physicians (including x-rays, MRIs, CT or reports)
  • a list of your current and/or recent medications
This information will be very helpful to your new physician and make your visit go much smoother.

Please be sure to arrive 10-15 minutes early in order to fill out any additional paperwork which may be required prior to your visit.

Appointment Cancellation Policy
24-hours notice is required if you need to cancel or reschedule your appointment

If you need to reschedule or cancel your appointment, please call at least 24-hours in advance and we will gladly accommodate your schedule.  

This policy is in place to ensure that all our patients can get timely appointments with their doctors.
